Hello.
Improving the essence of the skin with care,
Medical Director, Mind Dermatology
I am Hongseon Lee, a dermatologist with 17 years of experience.
"Between Ulthera and Thermage,
if you could only choose one,
which would be better?"
Many patients ask this during consultations.
But in fact, there isn't a single definitive answer to this question^^
For some people Ulthera suits them better, and for others Thermage is a better fit.
They both fall under the category of lifting procedures, but they work on different tissue layers. That said.
I will now explain how to distinguish between the two based on certain criteria.
If you're wondering which of the two would suit you better, today's post should be helpful.

Comparing how Ulthera and Thermage work
Doctor's criteria for differentiation: a complete summary!
In some cases, combining both may be preferable!
1. Comparing how Ulthera and Thermage work
The two procedures use different types of energy and stimulate different depths.
Thermage uses monopolar radiofrequency energy to deliver heat broadly and uniformly across the dermal layer rather than concentrating it at a single point.
When heat is applied in this way, collagen fibers immediately contract, restoring firmness, and that stimulation induces new collagen production.
In this process, improvements in skin texture and elasticity, fine wrinkles, and porescan be expected.
Ulthera focuses ultrasound energy precisely into the deeper layers of the skin, like concentrating sunlight with a magnifying glass.
Its main purpose is to reach the SMAS layer that supports the skin, contract the fascial layer, and improve sagging contours. That is its primary objective.
So it is a good option to consider for people whose facial lines have overall loosened and become less defined as aging progresses.

2. Doctor's criteria for differentiation: a complete summary!
Once you understand the mechanism, the criteria for choosing become clearer^^
For people whose face has a moderate amount of fat and sagging in the direction of gravity is the main issue, Ultheramight be the first option to consider.
Ultrasound energy that can reach the SMAS layer contracts the structures between the skin and muscle, helping create a firmer contour line.
Conversely, when the face does not have much fat but the skin is becoming thinner and issues like enlarged pores, rough texture, and fine lines are beginningto appear,
a Thermage treatment, which delivers uniform thermal stimulus to the dermis to induce collagen, may be more suitable.
Because it does not penetrate as deeply and acts mainly on the dermal layer, it can induce relatively stable effects even when the skin is thin or subcutaneous fat is minimal^^
However, the definitive distinctions should be made through a precise skin assessment.
Our skin often differs internally from what is visible to the eye.
So even I do not make hasty judgments about which procedure is appropriate based on visual inspection alone.
I always rely on a detailed diagnostic assessment that can clearly check the skin internally to carefully recommend which procedure is more appropriate^^
3. In some cases, 'combination' may be better!
Sometimes rather than choosing strictly between Ulthera and Thermage, combining them can be a better choice.
This applies to people whose aging progresses differently in different areas.
If the jawline is gradually sagging while the skin around the eyes and forehead is getting thin, crepey, and rough,
trying to solve everything with a single procedure can leave you unsatisfied.
Ultherato address deep-layer sagging,
and Thermage to induce dermal tightening,
because different energies work at different layers simultaneously, you can expect a synergistic effectthat may be worth trying^^
I hope this helps you achieve more satisfying care!
Which of the two procedures suits you better Precisely diagnose your skin condition in person You can only know by actually trying it.
So rather than deciding 'I'll do this too!' based only on friends' stories or online reviews,
I want to suggest that it's better to visit a dermatologist in person, get a diagnosis, and have a consultation so you can make a more careful decision.
Since it can involve a considerable expense, please be sure to learn in detail what approach suits your skin before proceeding.
I sincerely hope today's post was helpful for those deciding between Ulthera and Thermage. With that, I will say goodbye!
I will treat you sincerely.

Question |
Answer |
|
Q. Is it okay to receive Ulthera and Thermage on the same day? |
A. Some skins can tolerate combined procedures, and some cannot. So it's a bit difficult to give a definite yes or no answer. If the skin is very sensitive, combining procedures can actually be more irritating. Therefore I recommend getting a diagnosis first and then deciding. |
|
Q. When do results appear after Ulthera or Thermage? |
A. Both procedures stimulate the skin's own regenerative functions to bring about change, so immediate effects do not appear. Changes typically appear gradually over at least 1–3 months. I recommend allowing sufficient time. |
|
Q. Do I need to receive it every year? How long do the effects last? |
A. It depends on individual skin condition, but many people consider maintenance on an annual basis. Because aging itself continues, these are not one-time procedures that provide permanent maintenance. It's recommended to have basic maintenance done. |
